Level 5 Nurse - RGN (Domiciliary Care)
20 hrs per week - £13.84 per hr
Location: St Oswald's Village
Responsible to: Village Manager
Responsible for: Care and support of Residents assessed as requiring a nurse led service
Overall aim of the Job:-
- To support residents in the Village in a manner that enhances the quality of their life without compromising individual’s freedom of choice.
- To provide professional leadership and support to the staff.
- To ensure professional support to Residents and relatives.
- To take particular responsibility for an area of work as agreed with the Manager.
Key Responsibilities Include:-
- Responsibility for assessment, planning, overseeing of implementation and the evaluation of the needs of Residents.
- Developing individual care plans according to NMC Guidelines, CSCI Guidelines for nurse agencies and the philosophy of The ExtraCare Charitable Trust.
- To promote, at all times, the health and wellbeing of Residents.
- To always act in the best interests of the Resident, observing confidentiality and dignity at all times, especially outside the Scheme / Village environment.
- To take the clinical lead with individual Residents in respect of tissue viability, nutrition, continence, medication management, phlebotomy, diabetes management, wound management, pain management and palliative care, as required.
- To exercise this responsibility through participating in the care planning and review process in conjunction with medical advice.
- Where the predominant need of an individual Resident relates to their mental health status, their clinical nursing care will be assessed, planned, implemented and regularly reviewed with the appropriate guidance from professionals trained in supporting clients with this particular health issue.
- To document any interventions, whether advisory or through direct care, in the support plan in line with professional NMC guidelines and provide accurate statistical information, as required.
- To refer to, and liaise with the appropriate specialist nurses linked to the PCT.
- Refer to and liaise as appropriate with GPs, hospitals and members of the wider therapeutic teams.
- To work at all times within the policies and procedures as described in the Beautiful Book.
- Set standards for care, informed through evidence based practice, and act as a role model at all times.
- To be an active part of the Home Management Team, directing, motivating and monitoring performance.
- Direct the interventions of the key workers (RSWs) to ensure high standards of all aspects of care.
- Monitoring performance and training needs through the Development Review process.
- To ensure each Resident has an active, varied and interesting life which respects individual choice and capability.
- To develop sound relationships and communicate effectively and regularly with Residents, relatives or significant others, and involve as fully as possible with all aspects of care.
- To work with teams to identify areas for improvement and plan solutions to problems.
- To continually seek to improve own knowledge and skills.
- To act as a buddy (mentor) to new or less experienced staff.
- To always work within the professional NMC guidelines for practice and behaviour.
- To ensure that requirements of the Commission for Social Care Inspection are met in relation to nursing care.
- To encourage participation of Residents, wherever possible, in the running of the Scheme / Village. To ensure the safety of Residents whilst providing a comfortable and stimulating environment.
- To work with Team Leaders and support staff on an individual and group basis by regular meetings and exchange of information.
- To bring to the attention of the Manager matters affecting the work and welfare of staff.
- To work with Team Leaders to identify training and development needs of staff in respect of nursing care.
- To ensure that supplies are maintained and equipment is checked regularly.
- To attend regular meetings with the Manager to discuss matters relevant to the Scheme / Village and participate in the evaluation of their effectiveness.
- To work closely and liaise with Well-Being Advisor regarding all nursing issues.
Other:-
- All other Residents, not receiving level 5 nursing care, will call on community services i.e. G.P’s and District Nurses when appropriate needs are identified.
- Flexibility is essential in this role as you will be expected to cover for absences and holidays.
- The nurse, as part of her/his role will be expected to do on call as part of her/his duties over the 24 hour period. This will be on a rota basis.
- To maintain NMC registration and current PIN number
- To undertake any other duties which may be required from time to time.
- This post is subject to a satisfactory medical clearance, POVA and CRB checks and two written references.
